On the Caregiver Portal

You can also complete the Caregiver survey on the Caregiver Portal which can be accessed via desktop or laptop computer.

To complete the Caregiver survey in the Caregiver Portal, follow these steps:

1. Login to the caregiver portal and select ‘Results’.
2. Select the green bar labeled ‘Places of Learning’ (located next to the learning rings above ‘Home’, ‘School’ and ‘Community’).
3. Select ‘Caregiver Survey’ from the drop-down menu.
4. Complete the survey questions truthfully and entirely.
5. Select ‘Submit’.
